Alameda County Health: Housing & Homelessness Services is committed to improving health and housing outcomes for residents experiencing homelessness or housing instability across the county. At the heart of this effort is the Alameda County Health Care for the Homeless (ACHCH) program, a federally funded health center program that works to ensure access to culturally informed, whole-person health care, and housing support for people who are homeless or at risk of homelessness. ACHCH coordinates a network of local health centers and community partners to provide primary care, behavioral health, dental and optical services, mobile and shelter health, case management, and outreach services — all offered at no cost to those who cannot pay.

In partnership with the County’s Office of Housing & Homelessness Services and other community providers, the program also plays a key role in connecting people to emergency shelters, housing resources, benefits assistance, and coordinated support systems designed to help individuals and families stabilize and transition into long-term housing solutions. 

As winter approaches, Alameda County has again compiled its Winter Emergency Shelter Resources for the 2025-26 season — a critical lifeline for people experiencing homelessness during cold, wet, and inclement weather. This updated list includes county-wide winter shelters and warming centers that provide safe, dry places to stay throughout the season, with details on locations, operating dates, and how to access services. The list also highlights steps for connecting with emergency shelter and housing assistance through local support systems.
